In order to obtain a Birth Certificate you must complete an Application form. Forms are available through this website or by mail. The Fee is $23.00 per Certified Copy. Mail requests are accepted with an enclosed cashier’s check or money order payable to Wood County Clerk. No personal checks accepted. E-Mail requests are not accepted. The applicant must enclose a copy of his or her Driver’s License or state issued I.D. card. Birth records are closed and are only available to immediate family members or the individual named on the certificate. Genealogy researchers may obtain the record if the record is over 75 years old.
