In order to obtain a Death Certificate you must complete an Application form. Forms are available through this website or by mail. The Fee is $21.00 for the first certified copy and $4.00 for each additional certified copy ordered at the same time. Mail requests are accepted with an enclosed cashier’s check or money order payable to the Wood County Clerk. No personal checks accepted. E-Mail requests are not accepted. The applicant must enclose a copy of his or her Driver’s license or a state issued I.D. card. Death records are closed records and are only available to immediate family members. Genealogy researchers may obtain the record if the record id over 25 years old.
